The results come back within normal range, the physician offers reassurance, and the patient leaves no closer to understanding why their hair is falling out.

This scenario plays out thousands of times each week across the United States. The problem is not negligence on the part of primary care providers—it is a fundamental mismatch between what general medicine considers a diagnostic threshold and what dermatologists actually need to understand hair follicle behavior. The gap between those two standards can cost patients months of ineffective treatment and considerable financial outlay.

Why General Bloodwork Falls Short for Hair Loss

Standard laboratory panels are designed to identify disease states, not subclinical deficiencies or functional imbalances that fall within technically acceptable ranges. A ferritin level of 15 ng/mL, for instance, may satisfy a general practitioner's reference range but is clinically insufficient for hair follicle support. Most dermatologists and trichologists consider ferritin below 40 ng/mL—and ideally above 70 ng/mL—a meaningful contributor to diffuse shedding, particularly in women with telogen effluvium or androgenetic alopecia.

This distinction matters enormously. A patient told their iron levels are "normal" may begin an expensive topical or oral treatment regimen without addressing the foundational deficiency that is perpetuating their hair loss. Correcting that deficiency alone can, in many cases, substantially reduce shedding over a three-to-six-month period.

The Iron Cascade: It Is Not Just Hemoglobin

Iron-related hair loss requires more than a hemoglobin or hematocrit reading. Dermatologists typically request a full iron panel that includes serum ferritin, serum iron, total iron-binding capacity (TIBC), and transferrin saturation percentage. The standard TSH test measures thyroid-stimulating hormone, which reflects the pituitary gland's signal to the thyroid rather than actual circulating thyroid hormone levels. For hair loss evaluation, this is a starting point, not a conclusion.

Dermatologists typically extend thyroid assessment to include free T3 and free T4—the active and precursor forms of thyroid hormone—as well as reverse T3 (rT3). Reverse T3 is an inactive metabolite that can occupy thyroid hormone receptors without producing the cellular effects of active T3. These inflammatory indices help distinguish between androgenetic hair loss and immune-mediated conditions that require entirely different treatment protocols.

For patients with suspected androgenetic alopecia or hormonal hair loss, a comprehensive hormonal evaluation extends well beyond a basic testosterone reading. Dermatologists typically order:

Zinc, Vitamin D, and the Micronutrient Blind Spot

While a separate discussion of micronutrients exists within the broader diagnostic conversation, it bears noting that zinc and vitamin D are frequently omitted from general wellness panels despite their well-documented roles in hair follicle cycling. Zinc deficiency has been associated with diffuse alopecia and poor treatment response to topical minoxidil. Vitamin D deficiency—extraordinarily prevalent in the United States, particularly among individuals with limited sun exposure—has been linked to both telogen effluvium and alopecia areata through its role in follicular immune modulation.

Navigating the American healthcare system to obtain specialized laboratory work requires preparation. A few practical strategies can improve your likelihood of receiving a thorough evaluation:

Request a dermatology referral early. Primary care providers often defer to specialists for complex presentations. Framing your concern as a persistent, progressive issue rather than a cosmetic complaint increases the likelihood of a referral.

Arrive with a symptom timeline. Documenting when shedding began, any associated life events (illness, surgery, significant stress, dietary changes), and any treatments already attempted allows a dermatologist to prioritize the most diagnostically relevant tests.

Ask specifically about the tests discussed above. Using precise terminology—"I would like my ferritin, transferrin saturation, free T3, reverse T3, and DHEA-S evaluated"—signals medical literacy and often results in a more thorough diagnostic conversation.

Consider direct-to-consumer laboratory services. Companies such as Quest Diagnostics and LabCorp offer patient-initiated testing in most US states, allowing individuals to obtain specific panels without a physician order. While this approach bypasses insurance coverage, it provides access to results that can then be reviewed with a specialist.

The Cost of Skipping Comprehensive Diagnostics

The economic case for thorough laboratory evaluation before initiating treatment is straightforward. A comprehensive panel—including the thyroid, iron, hormonal, and inflammatory markers described above—typically costs between $150 and $400 out of pocket, depending on the laboratory and geographic location. Compare that to six to twelve months of prescription medication or a hair restoration procedure that fails to address an underlying deficiency, and the diagnostic investment becomes self-evident.

More importantly, treatment protocols built on accurate laboratory data are substantially more likely to produce meaningful results. Dermatologists who specialize in hair loss consistently report that patients who arrive with complete baseline bloodwork move through the treatment decision process more efficiently and experience fewer therapeutic dead ends.

The information exists. The tests are available. The decision to pursue them—before committing to any restoration strategy—is one of the most evidence-aligned choices a hair loss patient can make.
